■520    ▼aDeath  is  a  part  of  life  that  can  cast  a  long  shadow  that  steals  the  precious  moments  of  opportunities.  However,  when  people  are  comfortable,  spiritually,  physically  and  psychologically,  they  frequently  find  the  inner  strength  to  connect  with  themselves  and  their  family.  They  also  find  moments  of  profound  meaning  in  their  final  journey  of  life.  Finding  such  meaning  and  comfort  is  one  of  the  VA  Healthcare  System's  goals  for  the  veterans  who  are  admitted  to  the  PC/Hospice  unit.  In  addition  to  meeting  JAHCO  requirements  for  quality  PC/Hospice  care  nationally,  the  VA  is  participating  in  a  survey  administered  by  an  independent  Bereavement  Family  National  Survey  company  which  is  conducted  quarterly.
■520    ▼aThe  purpose  of  JAHCO  and  the  independent  survey  is  to  ensure  that  we  follow  evidence-based  practices  in  providing  final  care  services  to  this  frail  population.  These  services  include  Chaplaincy,  that  is,  spiritual  care.  Information  from  JAHCO  and  the  National  Survey  indicates  a  need  to  improve  Chaplaincy  skills  and  clinical  pastoral  training  to  meet  our  veterans'  spiritual  needs.  With  this  need  in  mind,  this  research  was  created  to  explore  the  effects  of  adding  personalized  prayerful  ritual  to  traditional  pastoral  care.
■520    ▼aThe  purpose  of  this  research,  held  at  the  Department  of  VA  Extended  Care  Center  at  179th  Street  &  Linden,  St  Albans,  New  York  11425,  is  to  identify  how  Chaplains  can  help  PC/Hospice  veterans  and  their  families  to  create  a  personalized  prayerful  ritual  in  order  to  bring  coping  strategies  to  spiritual  challenges.  The  spiritual  needs  that  I  wish  to  specifically  address  are  the  following:  reconciliation  through  forgiveness,  gratitude,  and  acceptance  of  self  and  families.
