요약 : Understanding multilevel pathways to functional outcomes of work, social functioning, and independent living in schizophrenia is important both for increasing knowledge of the etiology of the disease and for improving psychosocial treatment. This study proposed and tested the (1) temporal invariance and (2) causal direction of a multilevel path model linking functional outcomes in schizophrenia to measures of neurocognition, social cognition, social competence, and social support. In a previous study, this model had excellent fit to the data at baseline and was supported by strong theory and previous research. In the current study, the model was tested using data from a quasi-experimental longitudinal study in which participants were recruited as admitted to community-based urban rehabilitation programs. Ninety-seven of these individuals with schizophrenia or schizoaffective disorder were assessed at both baseline and 12 months with a battery of neurocognition, social cognition, and psychosocial measures. Baseline and 12-month paths were constrained to equal, then partially relaxed to test invariance of the model over time. To test causal direction, three methods were adapted to the multivariate path model because regression to the mean is a particular problem of two-wave data: cross-lagged panel correlation, multiple regression, and time-reversed regression. Results demonstrated very good model fit to the data at both time waves with slight weakening of the indices over time, and suggested that a more parsimonious model might cross-validate well in another sample. However, strong theory and other research argue for retaining the full model as proposed. Also, modest support was found for the overall causal direction of the model, with some indication of possible specification error or bidirectionality. Methods to improve the model are discussed, as well as implications for psychosocial rehabilitation, multilevel modeling, causal inference, and future study.
