요약 : To date, few research studies have focused on the alcohol and drug addiction treatment workforce. This study examines the addiction counselor's work environment, taking into consideration the impact of the facility-specific context on the behavior of the counselors employed within each facility, and relating these factors to the quality of care generated at the facility level. The questions guiding this research are: (1) how does perception of work environment vary at the individual counselor level? (2) how do perceptions of work environment vary across facilities? (3) do counselor perceptions of facility level work environment impact staff turnover? (4) is there an association between counselor perceptions of facility work environment and the quality of care produced at the facility level?

요약 : A combination of primary and secondary data was used. The primary data sources included the Facility Workforce Survey, and the Counselor Survey, collected from 54 Massachusetts Bureau of Substance Abuse Services (BSAS) licensed outpatient substance abuse treatment facilities. The secondary data consisted of client intake and enrollment data from BSAS. Quality of care was operationalized as the number of newly enrolled outpatient clients who subsequently enrolled in detoxification services within 90- or 180-days of enrollment.

요약 : Analyses included multilevel models to detect variation in counselor perception of the work environment within and between facilities, Poisson regression models to predict the number of vacancies and resignations at the facility level, and negative binomial regression models to predict the number of detoxification program admissions after a client enrolled in outpatient services.

요약 : The overall findings from this study suggest that significant variation in counselor perception of the work environment exist both within-facilities and between-facilities. Counselor level of burnout was significantly associated with work environment score, counselor rating of quality of care, and counselor overall job satisfaction. Counselor turnover was significantly associated with multiple facility characteristics, including facility focus and type of ownership. Last, this study was able to determine that there is an association between work environment and quality of care outcomes.

요약 : Reviewing the many detailed findings from this research, four dominant themes emerge: (1) the passion and commitment among the counseling staff working in the field, (2) the negative impact of burnout on perception of work environment, (3) the importance of understanding the impact of program level supports on staff turnover; and embedded within that, the recognition that counselors vary, and different counselor cohorts may require different types of supports to reduce turnover, and (4) the recognition that work environment does impact the quality of care.

요약 : Workforce issues within the field of addiction treatment, indeed across the entire spectrum of the health care delivery system, have moved into the spotlight and have taken on a level of urgency previously unknown; with all professions impacted by recent legislation concerned about the future. The overall findings from this study offer clear guidance and recommendations for policy makers, funders, and program administrators who are addressing workforce demographics, counselor needs, staffing trends, and other "big picture" workforce areas ripe for policy intervention.
